One(1)mobilization to the site is anticipated. • Overtime rate is applicable after eight(8)hours,on weekends and outside of normal hours(7am-4pm) No delays in the diligent execution of the work beyond the control of HEPACO are anticipated. • HEPACO will have free and exclusive access to the site during work hours. ■ Work will be performed in accordance with applicable federal, state and local regulations in effect as of this date. • Access to regulated areas will be restricted,as required by law. ■ Any required monitoring or testing will be provided by Owner. ■ Costs for a closure report are not included. • Costs for soil sampling and analysis are not included. ■ The Project Owner will retain title and generator status of waste materials, whether hazardous or non- hazardous, Generator will be responsible for additional costs for any non-conforming waste materials. • Visual clearances will be used as the work area clearance criteria, • Energy and Security Recovery Fee: A variable Energy and Security Recovery Fee will be applied to the total invoice.This fee will fluctuate with the DOE national average on-highway diesel fuel price;however, there will be no adjustment to the fee during an individual project.Fees will be calculated for an individual project based upon the DOE national average on-highway diesel fuel price on the first day of the project.In the event that the average price equals or exceeds$2.50 per gallon,a surcharge beginning at one percent (1%) will be applied. For every five-cent increase above $2.50 per gallon, the surcharge applied will increase by 0.5%.Should fuel prices drop below$2.50 per gallon,no surcharge would apply. Owner shall pay HEPACO for HEPACO's labor,equipment,materials,reporting and administrative tasks, services and other items furnished in performance of HEPACO's work until completion or until the earlier termination of the Proposal. Payment terms and conditions are in accordance to the existing contract#5702 between Orange County Government and HF:PACO dated August 19,2015. • HEPACO's will require a Purchase Order and/or signed Quote prior to project start. ■ This Proposal is valid for a period of thirty(30)Days.
