Client referred by DSS will be expected to <br /> sign at a minimum a release for Carolina Outreach to share attendance. <br /> f. Therapist will complete a Clinical Assessment at first appointment and then make <br /> recommendations for treatment. If Outpatient Therapy is the clinically appropriate level <br /> of care, Carolina Outreach Therapist will create a treatment plan with the client and offer <br /> individual and/or family therapy for a length of time that is appropriate based on <br /> diagnosis and symptom presentation, If Client referred is in need of a higher level of <br /> care, Therapist will make an attempt to refer to the appropriate service as is possible, <br /> considering funding source availability. <br /> g. If referred client does not engage at all after referral or initial assessment or does not <br /> show up to two sessions in a row, Carolina Outreach Therapist will discharge after two <br /> attempts to contact. <br /> h. Carolina Outreach will track reasons for discharge to include: lack of engagement, <br /> refused treatment,treatment goals met,and client referred for higher level of care. <br /> i. DSS will refer clients through the online referral page on the Carolina Outreach website. <br /> Carolina Outreach therapist will make initial contact to set up the first appointment. <br /> Ongoing, clients will be provided a contact number to communicate directly with the <br /> assigned therapist in regards to appointments. <br /> j. Additionally, the therapist can refer a client to Carolina Outreach for psychiatry <br /> consultation/medication management if this is an identified need. If the client has no <br /> income and IPRS funds are available, IPRS funds will be utilized. In all other instances, <br /> DSS will use funds to cover the costs of these appointments. <br /> 4. Duration of Services <br /> a. Term. The term of this Agreement shall be from July 1, 2019 to June 30,2020. <br /> b. Scheduling of Services. <br /> i) The Provider shall schedule and perform its activities in a timely manner. <br /> ii) Should the County determine that the Provider is behind schedule, it may require <br /> the Provider to expedite and accelerate its efforts, including providing additional <br /> resources and working overtime, as necessary, to perform its services in <br /> accordance with the approved project schedule at no additional cost to the <br /> County. <br /> iii) The Commencement Date for the Provider's Basic Services shall be July 1, 2019. <br /> 5. Compensation <br /> a. Conn ensation for Basic Services. Compensation for Basic Services shall include all <br /> compensation due the Provider from the County for all services under this Agreement. <br /> The maximum amount payable for Basic Services shall not exceed Sixty Thousand <br /> Dollars Dollars ($60,000.00). Payment for Basic Services shall become due and payable <br /> within thirty (30) days of Provider properly invoicing County.
