BILLING <br /> The Contractor will be responsible to bill the County within thirty (30) days for services rendered the <br /> previous month <br /> A. OFOA <br /> Location/site/address, amounts listed in drums or gallons, testing date and all applicable details <br /> must be listed upon the bill. <br /> B. HHW <br /> Location/site/address, date collected, date processed, and detailed amounts listed in types of <br /> materials pounds and all labor must be included in each contract. <br /> 5. REPORTING <br /> Contractor shall submit monthly and annual reports to the County by the tenth (loth) working day of <br /> the month. The required monthly reports shall be submitted electronically and include an invoice for <br /> the past months services, and shall include an itemized list of all charges to the County for materials <br /> used, materials delivered, cost per pound, and itemized pounds. In addition, the total pounds <br /> collected for each collection day listed by CESQG, and the overall total shall be itemized. <br /> A. Monthly Reports <br /> 1. Detailed Monthly Report: To be submitted with invoice for services, shall include a breakdown of <br /> material accepted by weight by Department of Transportation (DOT) Hazard Class, including the <br /> disposal method used to manage this material. The materials listed should be details and <br /> itemized. <br /> 2. North Carolina Hazardous Waste Facility Annual Report: To be submitted in the format as <br /> requested or required by the State. Currently, the State requires collection be segregated into the <br /> categories listed below. The categories listed may change based on State modifications. The <br /> Contractor shall also include the total pounds for each type as well as the treatment used for <br /> each type. Please remember recycling should be utilized whenever possible. <br /> a. Categories: Compressed Gases, Flammable Solids, Flammable Liquids, Electronics <br /> material, Oxidizing material, Poisonous material, Radioactive material, Corrosive <br /> material, Freon Cylinders, Hg and FIG debris, PCB Ballasts, Latex Paint, Alkyd Paint, <br /> Antifreeze, Used Oil, Oil Filters, Batteries, Regulated material, Non-regulated material, <br /> Propane, Other. <br /> 3. Monthly Invoicing: Invoices shall also be submitted in an electronic format. Tracking for HHW and <br /> OFOA should be handled separately. Data shall include, but not be limited to: <br /> - Materials collected <br /> - Material pounds/units/quantities <br /> - Material rates (if applicable) <br /> - Material costs (if applicable) <br /> - Material treatment methods including: <br /> 24 of 45 <br /> RFP No. 5255 <br />
