(hereinafter called the permittee or PCI), to <br /> operate a hazardous waste treatment and storage facility for the management of hazardous <br /> waste, located in Millington, Shelby County, Tennessee, at latitude 35' 16' 030" and longitude <br /> 89' 56' 039". The permittee shall be allowed to store and treat hazardous waste in containers, <br /> tanks, and miscellaneous units only in accordance with the terms of this permit. <br /> This permit is issued under the authority of §68-212-108. This permit, in conjunction with the <br /> HSWA portion of the RCRA permit, issued by the Environmental Protection Agency, constitutes <br /> the full hazardous waste permit for this facility. The permittee shall be required to investigate <br /> any releases of hazardous waste or hazardous constituents pursuant to this permit at the facility <br /> regardless of the time at which waste was placed in a unit and to take appropriate corrective <br /> action for any such releases. The permit also requires the pen-nittee to comply with all land <br /> disposal restrictions and air emission standards applicable to this facility and to certify annually <br /> that on-site generation of hazardous waste is minimized to the extent practicable. <br /> The permittee must comply with all terms and conditions of this pen-nit. This permit consists of <br /> the conditions contained herein (including those in any attachments) and the applicable <br /> regulations contained in Rule Chapters 1200-1-11 or 1200-1-14, as specified in the permit. <br /> Applicable regulations are those from Rule 1200-1-11-.06 or from Rule 1200-1-14-1'03, which <br /> are in effect on the date of issuance of the permit; for all other rules in Rule Chapters 1200-1-11 <br /> or 1200-1-14, applicable regulations are those in effect on the date of the issuance of this permit <br /> and any subsequent modifications to those rules as they become effective. <br /> Continuation, Transfer, Modification, Revocation and Reissuance, and Termination of this <br /> permit shall comply with and conform to Rule 1200-1-11-.07(9). <br /> This permit is based on the premise that the information and reports submitted by the permittee <br /> prior to issuance of this permit or prior to any subsequent modification to this permit are <br /> accurate. Any inaccuracies found in this information or infonmation submitted as required by <br /> this permit may be grounds for termination or modification of this permit and enforcement action. <br /> The Commissioner may modify this permit if information is received which was not available at <br /> 1 <br /> Sustainability at Work 45 <br />
