CONTRACTOR EMPLOYEES AND LABOR <br /> Contractor's employees shall be competent, reliable and able to perform duties under the Contract <br /> without assistance. Contractor shall have adequate labor, facilities, and equipment and inventory to <br /> handle the services as proposed in this contract. <br /> 5. PROPOSAL REQUIREMENTS <br /> Submitted proposal must follow the following order and format: <br /> 5.1 PROPOSAL SUBMITTAL <br /> Submitted proposal must follow the following order and format <br /> 5.1.1 Introduction <br /> This section must include a brief statement of Proposer's Company Background, Contact <br /> Information, and must include the signature of an individual who is authorized to bind the <br /> Proposer contractually(see section IV, subsection C). <br /> 5.1.2 Statement of Understanding of Services Sought by County - <br /> This section must include a statement of Proposer's understanding of the services being sought <br /> by the County, and include a description of Scope of Work for Recycling Composition Study. <br /> 5.1.3 Current and Past Experience <br /> This section must include a description of Proposer's current and past experience providing <br /> services similar to those the County seeks. This section must include a list of clients to whom <br /> similar services have been provided within the past three (3) years. The list of clients must <br /> include accurate name, phone number, and email address (when possible) of contact person. <br /> Current and/ or past clients may be asked by County to provide reference for Proposer. <br /> 5.1.4 Technical Expertise <br /> This section must include a description of Proposer's technical expertise in the conduct of <br /> Recycling Composition Studies. <br /> 5.1.5 Project Approach <br /> This section must include a complete description of Project Approach, including the proposed <br /> sampling methodology and the advantages and disadvantages of the approach as well as the <br /> proposed approach to assessing and evaluating contamination. <br /> 5.1.6 Pricin <br /> This section should contain pricing as requested in the table attached as Attachment F. <br /> 5.1.7 Financial Capability and Insurance - This section must include an indication of financial <br /> capability for handling services to be delivered, including Insurance Coverage. <br /> 5.2 PROPOSAL ACCEPTANCE <br /> Any incomplete proposal or proposal deviating from the required format may be eliminated by the <br /> County. <br /> 5.3 SELECTION PROCESS <br /> A selection committee shall be established by the County to review responses. The following will be the <br /> screening criteria. Order below is not indicative of priority. <br /> • Overall Experience (25 points) . Proposer shall provide a list of similar projects <br /> including location, scope of service, date work performed, project cost, client's name <br /> and address, contact person and telephone numbers for a minimum of five previous <br /> clients within the last 10 years.
