Does the facility have a Resident’s Council? <br />Family Council? <br />yes <br />no <br /> <br /> Areas of Concern Yes/No/NA Exit Summary <br />Are there resident issues or topics that need follow-up or review at a later <br />time or during the next visit? <br /> Negative attitude of many staff members expressed by residents and <br />relatives. <br />Delay in answering call bells-one resident reported being wet several hours <br />before being helped. <br />Lack of assistance getting in and out of bed. <br />Daily activity calendar in very small print. <br />A family member indicated no one comes to ask about or assists in attending <br />activities. <br /> Discuss items from “Areas of Concern” Section as <br />well as any changes observed during the visit <br /> Interim Nursing Administrator was very <br />receptive to concerns expressed. She has had <br />experience as a CNA before becoming a nurse. <br />She was aware of “attitude issues” and said she <br />has been working with staff individually and as a <br />group to address this. <br />She also is working on call bell response. <br />She indicated she was aware of activity concerns <br />and was working to make changes . <br /> <br />This Document is PUBLIC RECORD.
