PROPOSAL FORM <br /> The Proposal Form is bound in the specification booklet and is to be utilized for submission of bids . <br /> A copy of the Proposal Form is included with these minutes . <br /> Bids are to be effective for 60 days . <br /> Project time is 60 consecutive calendar days . <br /> V. INSURANCE <br /> Insurance limits are noted in the specifications . All insurance is to be in full force and effect prior to <br /> award of contract . <br /> VI. GENERAL CONDITIONS <br /> Bidders were advised to be aware of the definitions listed in the specifications . <br /> Any and all permits and inspections that may be required are to be obtained by the Contractor at no <br /> cost to the Owner . Bidders were advised to be aware of and comply with all applicable laws , <br /> ordinances , rules , and regulations which may affect performance of the work. Bidders were advised <br /> to check with Orange County and the Town of Hillsborough for any needed permits . <br /> 5thWall will be responsible for administration of the project . <br /> A detailed project schedule is to be prepared by the contractor prior to job start and updated on a <br /> monthly basis . The schedule is to show graphically by bar chart, CPM or other acceptable method <br /> all work items required to complete the project from start to finish within the allotted time frame . <br /> No payments will be approved without a Schedule of Values . The Schedule of Values must be <br /> submitted and approved prior to job start and updated monthly . Pay requests due in Designer ' s <br /> office by 20ffi of each month . <br /> Retainage in the amount of 10% will be withheld from each pay request and will not be released until <br /> all close - out documents have been received and approved . <br /> Changes in the work will be addressed through issuance of change orders . Field orders may be <br /> issued so as not to impede job progress . 5thWall is to be contacted regarding items suspected to <br /> require a change order . <br /> Form of contract to be utilized between Owner and Contractor is attached. <br /> VII.
