Do the residents appear neat, clean and odor free? YES 1-There was considerable staff and visitor <br />interaction with residents <br />2. Did residents say they receive assistance with personal care <br />activities? Ex. brushing their teeth, combing their hair, inserting <br />dentures or cleaning their eyeglasses? <br />YES <br />2- People said they were well cared for <br />3. Did you see or hear residents being encouraged to participate in <br />their care by staff members? YES <br />4. Were residents interacting with staff, other residents & visitors? <br />YES <br />4- There was considerable interaction including a <br />small group activity and following discussion with <br />lots of laughter <br />5. Did staff respond to or interact with residents who had difficulty <br />communicating or making their needs known verbally? YES 5- Especially notable in Memory Care, everyone <br />was greeted by name, hugs and comments <br />6. Did you observe restraints in use? NO <br />7. Did residents describe their living environment as homelike? YES 8- All but one, who said she liked it at The <br />Stratford, but missed home. (She has been there <br />for 4 years). <br />9. Did you notice unpleasant odors in commonly used areas? NO 9- The entire facility was clean, bright, and <br />smelled fresh. Fresh flowers were in various <br />areas including on the lunch tables. <br />10. Did you see items that could cause harm or be hazardous? NO <br />11. Did residents feel their living areas were too noisy? NO <br />12. Does the facility accommodate smokers? <br />Where? XX Outside only <br />YES <br />13. Were residents able to reach their call bells with ease? 13- most residents were in the public areas after <br />lunch, so call bells were not noted. <br />14. Were residents asked their preferences or opinions about the <br />activities planned for them at the facility? <br />YES 15- New bulletin board displays were noted in <br />several locations, lots of color and introducing <br />activities and thoughts <br />16. Do residents have the opportunity to purchase personal items of <br />their choice using their monthly needs funds? <br />Can residents access their monthly needs funds at their <br />convenience? <br />YES <br /> <br />YES <br />16- The Chapel Hill Transit senior bus stops <br />outside the building and residents go shopping or <br />just bus riding. <br />17. Are residents asked their preferences about meal/snack choices? <br />Are they given a choice about where they prefer to dine? <br />YES <br /> <br />17- Several residents were eating ham <br />sandwiches instead of the fish entrée, those <br />eating the fish lunch said it was very good. <br />18. Do residents have privacy in making and receiving phone calls? 18- Did not observe
