3 <br /> <br />Brookdale could not provide the level of care that this resident needed and bring the doctor in to explain that the facility <br />could not provide the level of care that the resident needed. <br /> We are required to go once a quarter. If you would like Autumn to attend but does not count as an active visitor. <br /> Facilities are doing more through assessment and that is why are seeing more empty beds. <br /> Carol Woods (CCRC) – Gavin Locklear is the new administration and his office is in the health center. We rarely have <br />any complaints. <br /> Crescent Green-a couple allegations pending. <br /> Stratford and Joan Fields training was held at the facility. But the week before she did a Resident’s Rights and has to <br />complete and in-service if a facility was sighted for a finding. An Administration can request an in-service dignity, <br />customer service. Good Responses on Resident Activities. <br /> Cedar Grove and Charles House are on the schedule to meet before the May meeting. <br /> Livewell- she and Joan went to Coker Hill and one of their oldest resident had passed. <br /> Livewell-Pauline had a couple deficient have not been finalize “IDR” Process is still pending. <br /> Signature is a skilled nursing facility. <br />Orange County (OCDOA) <br />Department of Aging <br />Annie Deaver, <br />Aging Transitions Administrator <br /> A. Deaver stated all the social worker positions have been filled. <br /> “Connect More at Seymour” Group we have put this on pause and considering a “Caregiver Support Respite” Model <br />at Seymour and later Passmore Group in April. <br /> Aging Transitions Division: Seniors in Operation and a Volunteer Division. Internal nice transitional plan. <br /> Kathy Kerns is retiring officially this week and came back 2 days for 60-days and A. Gupta, Data Manager, will fill K. <br />Kern’s position and another position to fill from the outside. <br /> The entire <br />committee accepted <br />the Aging <br />Transitions <br />Administrator <br />Report. <br />Orange County Adult Day <br />Home Visitation Reports: <br /> <br />Orange County Family <br />Care Home (<6) Visitation <br />Reports: <br /> <br /> <br />• M. Zuber and group discussion regarding the disparity for seniors in Orange County. We meet 6 times a year maybe <br />we could meet 4 and have 2 meetings with staff at The Stratford and Crescent Green as we learned at the training that <br />one group met with the CEO of a facility to begin to engage change. Maybe spend more time at facilities that need <br />assistance as there is really nothing to improve at Carolwoods. Invite County Commissioners. The answer will be <br />money. Bring in more volunteers and interns from university and area schools. <br /> <br />• Some of the administrators are on the defensive if when we go in “ask the Administrator if there is anything that we can <br />help them with”: <br />• Livewell could like to have their residents attend sessions at the Seymour Center but transportation is the issue. <br />• Charles House has transportation but would like to know what events would be best for their residents to attend. <br />• M. Zuber stated how do we connect the resources with the communities we serve (pet therapy). W. Lang <br />suggested that some groups were formed to concentrate on resources for senior facilities. Master Aging Plan <br />discussion ensued. <br /> S. Little motioned to <br />approve the 3 <br />OCACH Facility <br />Reports January 2, <br />2019. <br /> G. Brown seconded <br />the motion. <br /> The full committee <br />accepted the reports <br />with minor edits as <br />needed.
