Areas of
Concern
Yes/No/NA Exit Summary
Are there resident issues or topics that need follow-up or review at a later
time or during the next visit?

Activities

Personal Care: Finger and toenail care needed

Staffing: Not enough especially night shift; noticed use of ear pods
despite NHA stress not to use personal cells while on duty
YES

YES

YES

YES

YES
Staff & residents very positive comments re:
new NHA: “he listens to me.” Remarkable
change. Two had been elsewhere and
mentioned “this was the best.”

Need activities more individualized for those
not interested in “group” sessions, e.g. bingo.
NHA confirmed clipping of finger nails was
within CAN scope of practice.; said he would
check about the resident’s care. He also
thought since a dental event had recently
happened , a podiatry visit was shortly
expected.

Several residents concerned not enough staff
present especially at night.
NHA emphasized he noted use of personal
devices; said he was working hard to change
habits; said staff knew the use of items would
be grounds for dismissal; agreed personal
habits difficult to change. Members
suggested might be helpful if rounds were
made by NHA on shifts other than day, also
on weekends/holidays.

General appearance throughout facility
noticeably improved; little odor, all visited
areas clean; staff observed using correct
hand hygiene. D, Housekeeping lauded by
members.

Staff grieving recent death of Director of
Nursing. Poster placed at front entrance with
photo of DON; signed by many staff. All were
encouraged to add comments. However,
none of the residents visited had noted any
decrease in staff interest for their needs.
Interim DON in place.

Top Copy is for the Regional Ombudsman’s Record. Bottom Copy is for the CAC’s Records.
