DocuSi n Envelope ID: B94F2D40-3B3C-47A7-86C2-6F1 B4F92C62C
E tnvironmentai 3omuons uroup
Leading people,companies and communities to live, Report with Protocols for Re m e d i a t i o n
work and prosper in a sustainable environment.
Assessment Protocol:
This report is based upon a non-destructive visual assessment, laser particle detection and material samples collected on
October 5, 2018. Components to be sampled were selected by the investigator. As mold can spread quickly under
optimal conditions, both visual mold and spores can vary from day to day. For this reason, Environmental Solutions
Group certifies that the mold counts contained in this report are valid on the date of inspection.
Protocols for remediation are valid for a period of thirty(30) days from the date this report was issued. Furthermore,
Environmental Solutions Group (ESG) certifies that material inside the building contains the spore counts as indicated in
the report. ESG is not able to certify that mold does not exist in areas that were inaccessible during the inspection
process or that mold spores are not present in the air of the building. Testing and analysis of this investigation only
pertains to damage from moisture and mold, and this report is intended to be relied on solely by the client. ESG's
liability is limited only to the fees paid by the client.
Brief History(based upon information provided by the client and the onsite assessment):
The two-level commercial building is located at 510 Meadowlands Dr., Hillsborough, NC. ESG was engaged to collect
samples throughout the building.
The building is currently occupied and was identified as being impacted by elevated humidity, moisture damage, and
microbial contamination resulting from a lack of proper conditioning and moisture infiltration around the windows and
lower level exterior walls. The occupants reported noticing a musty odor and suspect microbial growth was reported at
the HVAC cassette units and on furniture in some of the offices.
The client has requested an evaluation be performed so that ESG can develop a scope of work for remediation of
identified environmental hazards relative to moisture damage.
ESG's assessment of the property occurred on the afternoon of October 5, 2018.
Methods•
Moisture Meter: a Delmhorst 'Moisture Check' meter was used to test and verify the moisture content of building
materials. The meter utilizes the principle of electrical resistance, and uses wood or other hygroscopic materials as an
element in a circuit between two pins or electrodes that are inserted into the material.
Surface Sampling for Laboratory Analysis: representative bulk surface samples were collected using a sterile swab.
Results of the lab analysis are discussed below.
Air Sampling for Laboratory Analysis: samples were collected using a Micro-5 cassette for a 25 Liter collection of total
spores, viable and non-viable. A Buck L-5 sampling system was used for sample collection. ESG's Buck pumps are
serviced and calibrated by the manufacturer and were calibrated prior to and during the assessment with a Zefon air
flow rotometer. The sample collection locations were throughout the building, warehouse, and outdoors for
comparison purposes.
Environmental Solutions Group • 1000 N.
