DocuSign Envelope ID:815F210A-8B41-4FA5-9E00-6E68ADE7BD3A <br /> Community Empowerment Fund <br /> OCT 24 2017 INVOICE No.0107 INVOICE Prepared for OCPEH <br /> SCOPE OF WORK <br /> OC Connect was created as a first step towards Coordinated Entry(CE) and social service <br /> collaboration in Orange County for individuals at.Originally it was designed as an online <br /> platform to utilize the VI-SPDAT, prevention resources, and a community resource database to <br /> be a coordinated entry platform and organize homelessness services and resources.As of Feb <br /> 30th, 2015 it has been available through a training offered to individuals on the HOME <br /> Committee, and has served as the intake/entry process for referrals onto the HOME <br /> Committee. <br /> Since its creation: <br /> • 779 assessments have been made via OC Connect. <br /> • 344 of those individuals scored high enough on the VI-SPDAT to be referred onto the <br /> HOME Committee, prioritized for services according to their VI-SPDAT score, and <br /> connected to resources through the resource database. <br /> • 10,000 visits to the Resource Database have been made publicly via occonnect.info. <br /> • 3,000 people have accessed the Resource Database publicly via occonnect.info. <br /> • 700 resources are available on the Resource Database. <br /> As a community, Orange County moves towards a more connected and collaborative approach <br /> to homeless services,we are transitioning from assessment that only prioritizes services for the <br /> most chronic towards a fully coordinated entry system! <br /> 1 . System Access, Support and Services (annual) <br /> 1A. System Access & User Services <br /> This includes access to the platform via occonnect.info and the capacity to add unique logins <br /> for each partner agency to increase the security and integrity of the platform while expanding <br /> potential for inter-agency collaboration.Servicing of these logins (creating/editing/removing/ <br /> password-resets) is also included. <br /> 1 B. Data Verification and Updating <br /> CEF's team of over 160 Volunteer Advocates is trained to actively use the resource database <br /> and report any inconsistencies or necessary updates. In addition, CEF Staff works with a team of <br /> Advocates and CEF Staff to annually comb through the 700+ resources to verify/update the <br /> information. The team will also seek out and incorporate other databases and resources that <br /> have not yet been included. <br /> Community Empowerment • • • 11 1 • • <br />
