Three <br /> subcommittees conduct quarterly visits to assisted living facilities and annual visits to <br /> family care homes (defined as :56 residents). Information is shared regularly between <br /> committee members and the ombudsman via phone or e-mail. <br /> Brief Statement of Board/Commission's Assigned Charge and Responsibilities: <br /> We make official unannounced quarterly visits to each assisted living facility (6 total) in <br /> the county, and unannounced annual visits to family care homes (6 total) in the county. <br /> As advocates to residents, we listen to concerns, understand recommendations, and <br /> work to improve their lifestyle. One purpose of our visits is to foster collaborative <br /> relationships with facility administrators/staff to improve the quality of care. We then <br /> prepare and submit official reports on visits to these facilities as required by the NC <br /> Division of Aging & Adult Services. These reports summarize findings during visits to <br /> facilities, emphasize residents' rights, and highlight concerns with the quality of care. <br /> Additionally, we stay informed about living conditions and services for residents of long <br /> term care ("LTC") facilities. We advise the County Department of Aging and the Triangle <br /> J Ombudsperson about programs or ideas on improving care for residents in our <br />
