• Participating as a project leader in the selection of an assessment tool and the <br />development of a community training to conduct the assessments in the neighborhood <br />surrounding the Southern Human Service Center by March 2005. <br />• Compiling and analyzing the results of the community assessments, developing a report, <br />and distributing to Project Team members by April 2005. <br />• Participating with the Project Team in formulating recommendations and preparing the <br />final report, including graphics and layout of the final report by April 2005. <br />• Creating draft of trail map and producing a final trail map for publication by May 2005. <br />• Participating in the creation of the Active Seniors Project Toolkit by .June 2005. <br />The agreement with TJCOG is for a total of $16,540, all to be paid from the grant. The <br />remaining grant funds will be used by the Health Department to support the project outcomes. <br />FINANCIAL IMPACT: The one-time award of $19,710 will support the costs of the project, <br />including the agreement with TJCOG for the services of a Land Use Planning Manager, who <br />was part of the application team. No additional County funds are anticipated for the support of <br />this one-time project. <br />RECOMMENDATION(S): The Manager recommends that the Board approve the agreement, <br />subject to final review by staff and the County Attorney, and authorize the Chair to sign. <br />
