Agency Service Provision Attestation (Attachment 3): Attestation to the ability to <br />provide at least one certified person at the established level of service on each first <br />due apparatus. A description of the Agency’s capability to provide twenty-four hour <br />coverage, seven days per week for the requested level of service in the first due <br />area covered by the Agency. <br />a. For example, if an Agency were going to initiate an Advanced EMT service, <br />a description of how the Agency’s Advanced EMT's would be staffed to <br />ensure the first-out unit for medical calls is available 24/7. <br />b. This only applies to the Agency’s first due unit. If the first unit is dedicated <br />and a second unit for the same service area is dispatched to a second call, <br />the second unit would not be required to have an A-EMT on the apparatus. <br />4. Agency Training Coordinator Designation: <br />a. Each Agency must designate a training Coordinator. <br />b. This form (Attachment 4) will include the individual's name and preferred <br />contact information as well as their credentials / qualifications to ensure the <br />State's continuing education requirements are met. <br />c. This individual will be the "go to" person if the State or County request <br />access to provider’s continuing education records for auditing purpose. <br />d. The agency training Coordinator should work closely with the OCEMS <br />training Coordinator for mentorship and assistance with training. <br />e. Agencies have the option of naming a backup individual who will serve in <br />the instance that the primary training Coordinator is unable to provide the <br />service. <br />f. In order for agencies to conduct in-house continuing education instruction in <br />compliance with Orange County Educational Institution, Training <br />Coordinators must possess a Level I EMS Instructor credential from State <br />OEMS. <br />5. Agency Provider Initial Credentialing Process: Agency’s Plan for initial credentialing <br />a. Agency selecting EMT upgrade will: <br />i. Ensure all Agency EMT’s pass EMT protocol and medication testing. <br />ii. Ensure that providers document all Phase I core competency <br />requirements as described in attachment 5. <br />iii. Ensure completed initial Orange County Emergency Services <br />Credentialing TSOP at EMT level. <br />iv. Ensure completed documentation of process and submission the <br />OCES Training Coordinator for evaluation and Orange County <br />credentialing. <br />b. Agency selecting Advanced EMT upgrade will: <br />i.
