Our health coaching staff—comprised of registered nurses, health educators, tobacco <br /> treatment specialists, and other health care professionals—can provide coaching and <br /> resources around a broad range of health topics including: weight management, tobacco <br /> cessation, stress management, healthy eating, and chronic disease management. Our <br /> health coaches are trained in stages of change and motivational interviewing and <br /> coaching techniques that empower and motivate individuals to improve health behaviors <br /> and reduce their risk factors. <br /> The health coaching process works by 1) helping individuals recognize the impact of <br /> lifestyle choices on their health, 2) helping individuals understand the benefits of change, <br /> 3) identifying the barriers to change, 4) building skills and providing tools and resources, <br /> 5) helping individuals realize the need to take self-responsibility for their decision- <br /> making and behaviors, 6) helping a person build the self-efficacy and self-care skills they <br /> need to effectively self-manage their improvement goals, and 7) insuring follow through <br /> and change with effective accountability methods, 8) and supporting positive change. <br /> Coaching Protocol <br /> • Review individual health screening data, health risk factors, recent health <br /> behaviors and recent medical activity. <br /> • Assess health risk perceptions and readiness for change using the Health Belief <br /> and Stages of Change models. <br /> • Identify barriers to change, and work through resistance and obstacles. <br /> • Facilitate self-directed learning, skill-building, and goal setting. <br /> • Develop and monitor short and long range health goals (SMART goals). <br /> • Co-create an action or wellness plan to facilitate achievement of goals. <br /> • Schedule follow up sessions as needed. <br /> • Review any areas of concern with the Medical Director <br /> • Document progress, provide support, and deal with challenges <br /> • Evaluate for new focus/goals <br /> • Track participation <br /> Scheduling <br /> Scheduling will be offered through telephone, secure online portal, or a combination of <br /> both, depending on the needs of employees. We will provide a contact telephone number <br /> for employee assistance on scheduling, and send reminder phone calls and/or emails prior <br /> to screenings/coaching appointments. <br /> Privacy Guidelines <br /> • The UNC Wellness@ Work Program ensures employee confidentiality by <br /> maintaining a privacy wall between employees and organizational leadership. <br />
