EXHIBIT A <br /> Health Screening and Health Coaching Services for Orange County <br /> Wellnessna,Work Background <br /> Wellness @Work is a comprehensive worksite wellness program developed by the UNC <br /> Department of Family Medicine. We partner with organizations and their employees to <br /> create a culture of wellness, improve health behaviors, prevent and improve care for <br /> chronic disease, and reduce overall health care costs. Our staff includes physicians, nurse <br /> practitioners, registered nurses, tobacco treatment specialists, and health educators and <br /> coaches, all with expertise in population health management strategies, health behavior <br /> change, preventative medicine and chronic disease care. The program employs rigorous <br /> risk stratification of employees based on assessment of health behaviors and chronic <br /> illness. We create individualized wellness programs in collaboration with employers to <br /> meet the specific needs of their employees. <br /> The objective of the program is to provide data-driven interventions that: 1) identify and <br /> reduce high risk health behaviors, 2) prevent chronic disease and improve quality of care <br /> for chronic disease, 3) reduce unnecessary health care utilization and cost, 4) create a <br /> culture of wellness within an employee population, 5) reduce absenteeism and increase <br /> employee productivity, 6) provide routine data to employer to assess program <br /> effectiveness. <br /> Health Screening <br /> The staffing model for daily health screening sessions will include two registered nurses, <br /> one phlebotomist, and one to two administrative assistants to manage employee check-in <br /> and logistics. We will work with the County to determine time and location for multiple <br /> screening sessions. Individual employee screenings will last a maximum of 20 minutes. <br /> We will provide staffing, equipment and supplies to perform non-fasting standard health <br /> screenings for the following factors: Total cholesterol, HDL, LDL, triglycerides, glucose, <br /> blood pressure, BMI (Body Mass Index) height, and weight. Cholesterol and glucose <br /> levels will be measured by finger stick blood draw method, and individualized feedback <br /> will be provided to employees on site. UNC Family Medicine, Wellness @Work is CLIA <br /> certified. <br /> Health Coaching <br /> Following the health screening process, we will identify employees who are at moderate <br /> or high risk health levels and invite them to participate in individualized one-on-one <br /> health coaching sessions lasting up to 30 minutes each. <br />
