31 <br /> Given the fact that the overall Risk Mitigation and Housing Displacement Fund has a <br /> nominal allocation and since the Department of Social Services (DSS) and the Towns of <br /> Carrboro, Chapel Hill and Hillsborough have similar programs, the Housing and Community <br /> Development Department (OCHCD) will be very careful not to duplicate the resources and <br /> therefore will only provide assistance for individuals and families that live within the <br /> unincorporated areas of Orange County, as well as in the Town of Efland and portions of <br /> Mebane that are in the County except in the case of referrals from Coordinated Entry and the <br /> Criminal Justice Resource Department. These referrals will be accepted regardless of their <br /> location in the County. In addition, the County will not co-mingle these funds with assistance <br /> provided under the Rapid Re-Housing Program. <br /> Applying for Assistance: <br /> Individuals requesting assistance must be referred by any of the local service providers. <br /> In making the referral, local service providers should make a written request for assistance on <br /> their official letterhead. The written request should describe the need for assistance and any <br /> pertinent background information on the client, persons living in the household, and <br /> information on the identified housing unit or complex. The following additional documentation <br /> should be attached to the written request: <br /> • Signed consent by the client to release information to Orange County, this includes <br /> both the Housing and Community Development Department (OCHCD) and DSS, as <br /> applicable. In some cases, DSS and OCHCD may exchange information in order to <br /> provide a comprehensive package of assistance. <br /> • Documentation verifying total household income. <br /> • Copies of the lease or other information from the landlord or property management <br /> company that identifies that the client has been approved to live at the identified unit <br /> and amount of funds needed. <br /> • Statement or invoice from utility provider, as applicable. <br /> Displacement Mitigation: <br /> Assistance under this activity area is only available when there is an urgent community <br /> housing need and the displacement of residents occur, i.e. mobile home park closures. Funds <br /> can be used to provide limited direct assistance to affected individuals and families at or below <br /> eighty percent (80%) of the Area Median Income. Persons requesting assistance must be <br /> participating in the County's Displacement Mitigation Assistance Program (D-MAP) and working <br /> with EmPOWERment, Inc., the vendor handling Relocation Coordination Services for the <br /> Displacement Mitigation Assistance Program (D-MAP). Available assistance includes payment <br /> of security deposits, utility connections, etc. <br /> The information contained in these pages represents an overview of the Risk Mitigation and <br /> Housing Displacement Fund.
