8022.1 <br /> Appendix 12c 41 <br /> ATTACHMENT C <br /> CERTIFICATION REGARDING DRUG-FREE WORKPLACE REQUIREMENTS <br /> I. Instructions for Certification <br /> a. By submitting this application, the applicant it providing the <br /> certification set out below. <br /> b. The certification set out beta is a material representation of fact <br /> upon which reliance will be placed if HUD determines to award a <br /> grant to the applicant. If it is later determined that the <br /> applicant knowingly rendered a false certification, or otherwise <br /> violates the requirements of the Drug-Free Workplace Act, HUD, in <br /> addition to any other remedies available to the Federal Government, <br /> say take action authorised under the Drug-Free Workplace Act. <br /> 2. Certification Regarding Drug-Free Workplace Requirements <br /> a. The applicant certifies that it will provide a drug-free workplace <br /> by: <br /> 1. Publishing a statement notifying employees that the unlawful <br /> manufacture, distribution, dispensation, possession or use of a <br /> controlled substance is prohibited in the grantee's workplace <br /> and specifying the actions that will be taken against employees <br /> for violation of such prohibition; <br /> 2. Establishing a drug-free awareness prograe to inform employees <br /> about: <br /> i. the dangers of drug abuse in the workplace; <br /> ii. the applicant's policy of caintaining a drug-free <br /> workplace; <br /> iii. any available drug counseling, rehabilitation, and <br /> employee assistance programs; and <br /> iv. the penalties that may be imposed upon employees for drug <br /> abuse violations occurring in the workplace; <br /> 3. Making it a requirement that each employee to be engaged in the <br /> performance of the grant be given a copy of the statement <br /> required by paragraph 1; <br /> 6/90 12c-4 <br />
