This agreement allows the county to accept the funds from UNC <br />Hospitals. <br />Significant accomplishments of this effort and the coordinator in just the past two years include: <br />• Completion of the 2003 Community Health Assessment with participation from more than <br />700 County residents, <br />• Publication of Orange County Community Health Assessment Executive Summary. <br />• Securing grants for partners in specific high priority areas. Youth tobacco prevention <br />project; milk machines in schools, six mini-grants to community agencies, teen <br />pregnancy prevention grant funds, funding for coordinator position, support for family fun <br />and fitness day. <br />• Collaboration on publication of county-wide recreational map. <br />• Collaboration on child abuse and prevention community agency trainings and brawn bag <br />lunch series, <br />• Developed and conducted the first Family Fun and Fitness Day in October 2003 in <br />Chapel Hill, <br />• Published health information for employers in the Chapel Hill Chamber newsletter. <br />• Successful completion of statewide recertification of Healthy Carolinians in 2004. <br />• Coordinates five committees each with 3-25 members in high priority areas identified in <br />the community assessment, <br />FINANCIAL IMPACT: For the current fiscal year, the salary and benefits far the position are a <br />total of $49,337 and are being funded by the following sources: $15,000 from the Kate B. <br />Reynolds Foundation funds; $10,992 from UNC Hospitals; and $24,668 from the County <br />general fund, The positive balance of funds will cover the 2% cost of living increase authorized <br />for October 11, This is the second time UNC Hospitals has been able to identify funds in <br />support of this position. <br />RECOMMENDATION(S): The Manager recommends that the Board approve the agreement, <br />accept the funds in the amount of $10,992 for FY 2004-2005 from UNC Hospitals, and <br />authorize the Chair to sign the agreement subject to final review by staff and the County <br />Attorney, <br />
