Certify in writing within five (5) business days that all personally identifiable data <br /> stored on local servers, backup servers, backup media, or other media have been <br /> permanently de-identified. <br /> 6. Disposition of Data at Termination of Data-Contributing Organization(s). In the event of the <br /> termination of the Master Data-Sharing Agreement between UWGT and one or more, but not all, of <br /> Orange County FSA's data contributing organizations, UWGT shall provide appropriate subsets of <br /> Master Data Set to the relevant data-contributing organization(s)terminating their participation in <br /> the Agreement. <br /> 7. Disposition of Data upon Revocation of Consent by Client or Client's Guardian. In the event of <br /> the revocation of consent by a client or the legal guardian of a client, UWGT shall: <br /> i. Provide a complete copy of the client's personal records upon request <br /> ii. De-identify any personally identifiable data within the site, entered during this <br /> agreement and <br /> iii.
