12 <br /> <br /> 1 <br />In order to receive Disabled/Disabling Condition 2 <br />preference, the Local Rent Supplement Program 3 <br />applicants must present evidence of disabling 4 <br />condition diagnosis with one or more of the 5 <br />following conditions: substance use disorder, 6 <br />serious mental illness, developmental disability as 7 <br />defined by the Department of Housing and Urban 8 <br />Development (HUD). 9 <br />A disabling condition is defined by HUD as: 10 <br />1. A physical, mental, or emotional 11 <br />impairment which is expected to be of long-12 <br />continued and indefinite duration, 13 <br />substantially impedes an individual’s ability 14 <br />to live independently, and of such a nature 15 <br />that the disability could be improved by 16 <br />more suitable conditions; 17 <br />2. A developmental disability as defined in 18 <br />section 102 of the Developmental 19 <br />Disabilities Assistance and Bill of Rights 20 <br />Act; 21 <br />3. The disease of acquired 22 <br />immunodeficiency syndrome or any 23 <br />conditions arising from the etiological 24 <br />agency for acquired immunodeficiency 25 <br />syndrome; or 26 <br />4. A diagnosable substance abuse disorder. 27 <br /> 28 <br />Evidence of this criterion must include one of the 29 <br />following: 30 <br />a. Written verification of the 31 <br />condition from a professional 32 <br />licensed by the state to diagnose 33 <br />and treat the condition; 34 <br />b. Written verification from the Social 35 <br />Security Administration; 36 <br />c. Copies of a disability check (e.g., 37 <br />Social Security Disability Insurance 38 <br />check or Veterans Disability 39 <br />Compensation); Intake staff (or 40 <br />referral staff) observation that is 41 <br />confirmed by written verification of 42 <br />the condition from a professional 43 <br />licensed by the state to diagnose 44 <br />and treat the condition that is 45 <br />confirmed no later than fourteen (14) 46 <br />days of the application for 47 <br />assistance and accompanied with 48 <br />one of the types of evidence above; 49 <br />or other documentation approved by 50 <br />HUD. 51
