11 d. Verification that the Local Rent 1 Supplement Program applicant met one of 2 the seven conditions under the definition of 3 Homelessness as evidenced by one of the 4 following: 5 i. Source documents that evidence 6 one or more of the conditions; 7 1. A Homeless/At Risk 8 Homelessness Certification 9 Form completed by a service 10 provider. 11 2. A written statement by a 12 relevant third party or the 13 written certification by 14 referring agency staff of the 15 oral verification by the 16 relevant third party that the 17 applicant meets one or more 18 of the conditions; or 19 3. A written statement by the 20 referring agency staff that the 21 staff person has visited the 22 applicant’s residence and 23 determined that the applicant 24 meets one or more of the 25 conditions or, if a visit is not 26 feasible or relevant to the 27 determination, a written 28 statement by the referring 29 agency staff describing the 30 efforts taken to obtain the 31 required evidence. 32 33 34 3. In addition, one or more of the following should 35 be provided: 36 a. A written documentation of observation 37 by an outreach worker; 38 b. A written certification by the household 39 seeking assistance that demonstrates that 40 the individual or head of household is 41 currently homeless and living in a place not 42 meant for human habitation, in an 43 emergency shelter, or a safe haven. 44 45 46 Senior/Elderly Disabled Certification A birth certificate or other official record of birth is 47 the preferred form of age verification (must be at 48 least 62 years of age or older). An original 49 document that provides evidence of the receipt of 50 social security retirement benefits is acceptable. 51
