11 I a. Source documents (e.g., notice of 2 termination from employment, bank 3 statement, health care bill showing arrears); 4 or 5 b. Written statement by referring agency 6 staff confirming that they spoke to a 7 relevant third party to verify homelessness 8 or at risk homelessness status; or 9 c. Written statement by referring agency 10 staff of the efforts taken to obtain 11 verification through source documents and 12 relevant third parties 13 d. Verification that the Local Rent 14 Supplement Program applicant met one of 15 the seven conditions under the definition of 16 Homelessness as evidenced by one of the 17 following: 18 i. Source documents that evidence 19 one or more of the conditions; 20 1. A Homeless/At Risk 21 Homelessness Certification 22 Form completed by a service 23 provider. 24 2. A written statement by a 25 relevant third party or the 26 written certification by 27 referring agency staff of the 28 oral verification by the 29 relevant third party that the 30 applicant meets one or more 31 of the conditions; or 32 3. A written statement by the 33 referring agency staff that 34 the staff person has visited 35 the applicant's residence and 36 determined that the applicant 37 meets one or more of the 38 conditions or, if a visit is not 39 feasible or relevant to the 40 determination, a written 41 statement by the referring 42 agency staff describing the 43 efforts taken to obtain the 44 required evidence. 45 46 47 3. In addition, one or more of the following should 48 be provided: 49 a. A written documentation of observation 50 by an outreach worker;
