10 <br /> 1 • January 14, 2019—Announce new OC-LRSP <br /> 2 • January 17, 2019— Hold Briefing for Service <br /> 3 Providers <br /> 4 • Application Period Opens— February 1, 2019 <br /> 5 • Application Period Closes — February 8, 2019 <br /> 6 • Reviewing Applications and Determining <br /> 7 Preliminary Eligibility— February 11- March 15, <br /> 8 2019 (Screening period may be extended due <br /> 9 to the number of applications received.) <br /> 10 • Conduct Briefings and Begin Issuance of New <br /> 11 Vouchers — March 25—April 30, 2019 <br /> 12 <br /> 13 SCREENING AND ASSESSMENT The OCHA requires that any agency making <br /> 14 referrals to screen applicants for eligibility based on <br /> 15 the guidelines outlined in this document and to <br /> 16 submit to the OCHA required documents included <br /> 17 in the Local Rent Supplement Program Application <br /> 18 Packet. <br /> 19 Completed documents will be used by the PHA to: <br /> 20 1. Determine applicant's eligibility <br /> 21 2. Select participants <br /> 22 3. Notify applicants of their obligations as an OC- <br /> 23 LRSP recipient <br /> 24 <br /> 25 Completed application and verifications must be <br /> 26 current within the last sixty (60) days. Referring <br /> 27 agencies and case managers must conduct a <br /> 28 thorough screening and assessment of each <br /> 29 individual's care needs. The screening will include <br /> 30 assessment of the individual's ability to live <br /> 31 independently. <br /> 32 In addition, the individual must not present a <br /> 33 danger to himself or others and the individual must <br /> 34 not require a level of care that is not offered by OC <br /> 35 Local Rent Supplement Program. <br /> 36 <br /> 37 EVIDENCE OF ELIGIBILITY Homelessness/At Risk Homelessness Certification <br /> 38 The Local Rent Supplement Program applicants, who <br /> 39 qualify under the definition of Homelessness, must provide <br /> 40 the following documentation at the time of application to <br /> 41 establish that they meet the required criteria: <br /> 42 1. The Local Rent Supplement Program applicant's <br /> 43 written certification that they have insufficient <br /> 44 financial resources and support networks <br /> 45 immediately available to attain housing stability <br /> 46 2. Documentation to verify that the Local Rent <br /> 47 Supplement Program applicant did not have <br /> 48 sufficient resources or support networks <br /> 49 immediately available to attain housing stability as <br /> 50 evidenced by one of the following: <br />
