DocuSign Envelope ID: EB294A71-C7D1-4EE4-8287-60A6149320D4 <br /> Connecting with Families: <br /> Last summer, we hosted two community events to deepen connections with the community <br /> and raise awareness about the Food For the Summer program: <br /> Food for the Summer Dick-off at Northside Elementary which included a cookout, bounce <br /> house, farmer's market and activities for families. <br /> End of the Summer Eclipse Party which included safe eclipse viewing, face-painting, games, <br /> food and more for 330— 350 members of the community. <br /> Connects Families and Children with Year-round services: <br /> Again this summer, Food for the Summer worked closely with TABLE -- providing <br /> information to families at meal sites, handing out and collecting applications for TABLE <br /> backpack deliveries and assisting with weekly backpack distribution. <br /> e) Describe the community need or problem to be addressed in relation to the Ch_ apel <br /> Hill Human Services Needs Assessment, Orange County BOCC Goals and <br /> Priorities, Town of Chanel Hill Council Goals, Carrboro Board Priorities, or other <br /> community priorities (i.e. Council/Board Goals). Reference local data (using the <br /> provided links, i.e. Chapel Hill Human Services Needs Assessment) to support the <br /> need for this program. <br /> The Food For the Summer program addresses several items identified in the Human <br /> Services Needs Assessment including: <br /> Providing healthy meals to children and youth at no charge on weekdays <br /> throughout the summer break <br /> Providing free activities that positively engage children and youth from at-risk and <br /> low-income households <br /> f) Who is your target population of individuals to benefit from this program and <br /> how will they be identified and connected with the program? <br /> Our target population is the 3,400 children and youth in the CHCCS district who <br /> qualify for Free & Reduced Lunch. We use data provided by the school district to <br /> identify meal sites that are walkable for identified populations. <br /> We do outreach through school social workers, community partners (IFC, PORCH, <br /> TABLE), and through the town's Housing & Human Services communication <br /> channels. We display posters in healthcare clinics and public places throughout <br /> Chapel Hill & Carrboro and send home flyers, translated in numerous languages, in <br /> children's backpacks as well as in PORCH donation bags and TABLE backpacks. <br /> g) Describe the credentials of the program manager and other key staff.
