Since we have only been operating the grant a few months, <br /> we do not yet have comprehensive data. However, the 14 funded vouchers allow us <br /> to serve a subset of the many more individuals who meet program criteria. This <br /> means we can provide a permanent home for 14 of Orange County's most vulnerable <br /> individuals and families experiencing homelessness. <br /> Through our partnership with UNC Healthcare and Freedom House, we work toward <br /> the goal of affordable healthcare, including additional healthcare options and <br /> additional mental health service options and additional substance abuse <br /> services. All of the clients in the Permanent Supportive Housing program suffer <br /> from some form of disability, and a main function of the program is making sure they <br /> are linked with the care they need. <br /> f) Who is your target population of individuals to benefit from this program and how will <br /> they be identified and connected with the program? <br /> Individuals and families who are homeless due to a lack of affordable housing, <br /> domestic violence, eviction, unemployment, low-paying jobs, mental illness, lack of <br /> support services, lack of social supports, medical debt, and/or substance use. <br /> Residents in this program experience a combination of these challenges, which <br /> makes them the most vulnerable, highest need residents in Orange County. We <br /> accept referrals from the Orange County Partnership to End Homelessness HOME <br /> Committee, which assesses vulnerability using the Vulnerability Index— Service <br /> Prioritization Decision Assistance Too[. <br /> g) Describe the credentials of the program manager and other key staff. (Ex. Identify <br /> Program Manager and credentials, describe training provided to volunteers, etc.) <br /> Permanent Supportive Housing is staffed by one full-time Case Manager. She is <br /> supervised by IFC's Residential Services Director, who holds an MSW from UNC- <br /> Chapel Hill and has worked for IFC for more than 7 years. The Case Manager has <br /> also worked for IFC for more than 7 years, previously at both shelters. She is <br /> experienced in responding to complex needs of homeless families and is well- <br /> connected to existing resources. <br /> h) describe the specific period over which the activities will be carried out and include an <br /> implementation timeline. <br /> Activities are ongoing through FY19. <br /> i) Why is funding this program a good investment for the community? How does funding <br /> this program add value to the community?
