DocuSign Envelope ID:8332B88E-6069-4FC9-98F9-6D884A2A8399 <br /> Exhibit A <br /> Scope of Services for <br /> FPG After School 2017-2018 program <br /> forthe <br /> Family Success Alliance <br /> Purpose: Program support resources for FPG to provide after school programming for <br /> kindergarten and 1st grade students attending FPG, a zone 6 school. <br /> Budget: $2,500.00 <br /> • Funds will be used for teacher salaries for the program and developing instructional <br /> materials. <br /> Program Services: <br /> • Provide after school tutoring and homework help for kindergarten and 1st grade <br /> students at FPG who are either enrolled in FSA or who are assessed as below proficient <br /> in literacy <br /> • This proposal is for teachers to meet with a small group of 10-12 students for 1 % hours <br /> Monday-Thursday afternoons after school for two sessions running January 8-March 8 <br /> and April 9-May 24. <br /> • There will be one teacher per session. Each teacher will be compensated for 1.5 hour of <br /> teaching per day for a total of 14 weeks with an additional 10 hours of curriculum <br /> development. <br /> • These funds will allow FPG to expand afterschool programming which is now only open <br /> to students in grades 2-5. <br /> Outcomes <br /> • The students will be assessed using mClass at the middle of the year and again at the <br /> end of the year to measure growth. <br /> • Parent and staff surveys will also be administered to assess quality and potential <br /> program improvements. <br /> 1 <br />
