Name and address of at least three references including contact names for which <br /> the individual or firm has performed the same or similar services within the last <br /> three to five years. <br /> Evaluation Criteria <br /> Proposals will be reviewed and evaluated on the ability of the individual or firm to meet or <br /> exceed the requirements set forth in the specifications. <br /> Determination of the respondents to perform accordingly shall be made by an evaluation team <br /> consisting of representatives from Orange County Government, UNC Health Care, local K-12 <br /> school systems, and community stakeholders. <br /> Proposals shall be evaluated on the following criteria: <br /> 1. The quality and clarity of the proposal as well as the presentation materials will <br /> be considered a reflection of the individual or firm's ability to communicate to the <br /> public, County officials, and staff. <br /> 2. Consultants approach to analysis and plan. <br /> 3. Experience, both general and with projects of this type. <br /> 4. Quality of past work. <br /> 5. Ability to complete the project within budget. <br /> 6. Ability to meet the desired time line for the project. <br /> Submittal Instructions <br /> Please submit three copies (one clearly marked "original") of your response (printed duplex on <br /> recycled paper). Attach cost proposal to the original in a separate, sealed envelope. Also <br /> include an electronic copy of the proposal and cost proposal in .pdf format on a thumb drive, or <br /> cd.
