DocuSign Envelope ID:7B55B196-3912-4C1A-ADB4-A753058DFEFD XHIBIT A
PROVIDER'S OUTSIDE AGENCY APPLICATION
f) Who is your target population of individuals to benefit from this program and how will they
be identified and connected with the program?
The Arc provides supports and services to more than three hundred adults and
children with IIDD. People identified with IIDD are individuals that have autism,
Cerebral palsy, Down syndrome, epilepsy and individuals that are classified as
PDDNOS (Pervasive Development Delay Not Otherwise Specified).
Referrals for our services are made through Medicaid, Dept. of Health and Human
Services, LME's, (Local Management Entities such as Cardinal Innovations, Alliance)
schools, educational facilities and word of mouth. The Arc is among an already
small network of agencies providing support and services to individuals in the
community that is rapidly decreasing because of budget cuts, in spite of an ever
higher and ever increasing number of people needing our assistance.
g) Describe the credentials of the program manager and other key staff. (Ex. Identify Program
Manager and credentials, describe training provided to volunteers, etc.) The
Assistant Director of Social and Volunteer Programs is a Certified Volunteer
Administrator (CVANC), credentialed in the state of North Carolina, having completed
a one year course in volunteer management. The Assistant Director is also a
certified C&I (Communications and Interaction Strategies) training instructor. C&I
training focuses on healthy interactions with persons with IIDD. These trainings are
given to all Arc paid staff and Are volunteers in addition to the on-on-one training
they currently receive. Arc staff that oversee participant caseloads are certified
Qualified Professionals.
h) Describe the specific period over which the activities will be carried out and include an
implementation timeline.
All social programs and individual service are currently implemented and are offered
ongoing throughout the year.
i) Why is funding this program a good investment for the community? How does funding this
program add value to the community? (250 words OR LESS)
A growing body of research has found that the presence of social capital through
creation of social networks and communities has a protective quality on health.
Social capital affects health risk behavior in the sense that individuals who are
embedded in a network or community rich in support, social trust, information, and
norms, have resources that help achieve health goals. For example, a person who is
sick with cancer may receive information, money, or moral support he or she needs
to endure treatment and recover. Social capital also encourages social trust and
membership. These factors can discourage individuals from engaging in risky health
behaviors such as smoking and binge drinking.
Experts call this social capital; I call it my personal life experience. Attached is an
article I wrote for the local news that reflects on raising my son who has autism.
When he was diagnosed twenty-five years ago I was given this advice that I still find
pertinent today "Use every experience as a learning experience, keep your son
engaged in the community with typically developing children and most importantly
PROGRAM INFORMATION 1/26/2017 5:52:39 PM to c; e i o
