DocuSign Envelope ID:3457204A-7765-4E36-8B42-227F6F673AF6 XHIBIT A <br /> PROVIDER'S OUTSIDE AGENCY APPLICATION <br /> • We've been successful in strengthening our relationship with the Towns of <br /> Carrboro and Chapel Hill, and have formed a partnership with the Chapel Hill <br /> Youth Council. <br /> • We hosted our second annual youth art exhibition and auction to showcase over <br /> 50 works of art produced by local youth in partnership with YCP & art clubs at <br /> Chapel Hill-Carrboro area high schools <br /> • YCP, like the Youth Initiative, has conducted surveys to identify youth interests. <br /> Our discussions have suggested substantial overlap. We believe the programs we <br /> propose are important developments in responding to those needs. <br /> e) Other Pertinent Agency Information (Ex. Has the agency experienced any major changes <br /> in the past year? Is there a new Executive Director?Are there new initiatives?) Other <br /> Pertinent Information: Chapel Hill's 2020 Plan has as its stated goal to: Review and <br /> refine focus for youth services and begin planning for youth-centered facilities. <br /> Further, it calls for both raising youth awareness of the town's current programs and <br /> spaces, and collaborating and innovating to develop and deploy new programs and <br /> services. YCP is a community leader serving to achieve of these goals. <br /> f) Schedule of Positions (For Entire Agency) <br /> • Full Time Equivalent(FTE) staff will be noted as 1.00; half time as .50; quarter time as .25, etc. <br /> • Calculate a Full Time Equivalent for all recorded volunteer hours using the following: <br /> Total Volunteer Hours = Volunteer FTE <br /> 2,080 <br /> # of FTE - Full-Time Paid Positions: <br /> # of FTE - Paid Part-Time Positions: 3 <br /> # of Volunteers: 50 # of FTE -Volunteers: 1 <br /> g) Living Wage <br /> Does this agency pay permanent employees a minimum living wage? (Yes/No) Yes <br /> If yes, is this agency an Orange County Living Wage Certified Employer? No <br /> If no, please explain. <br /> *** YCP is a very small organization with limited resources for activities outside our core <br /> mission. Nonetheless, we have a very strong commitment to the concept of a living <br /> wage and substantially exceed living wages for all employees.
