Patient is a 63 year old male,conscious and breathing,complaining of nausea.The wife has called 911 <br /> stating that she feels something is wrong with her husband. <br /> Present on scene/General Impression: <br /> Husband and Wife couple—no family—no other persons—no animals <br /> Patient location:Patient reclining in chair recliner in the living room and has his hands over his upper abdomen. <br /> AVPU-Patient is ALERT and oriented to person,place,time,and event <br /> Skin pale,cool,and clammy.Approximate weight: 220 Ibs-No apparent distress <br /> Patient Complaint: <br /> Patient complains of slight nausea with one episode of vomiting. <br /> 1 SAMPLE History Obtained <br /> S: Nausea,clam my <br /> A: Na Known Drug Allergies <br /> M:Simvastatin,Omeprazole, Nitroglycerin tabs <br /> P: Hyperlipidemia,cholecystectomy, heart problem <br /> Familial history: Brother died from a heart attack at age 62.Father died from a heart attack at the age of 60. <br /> L: 3-day-old Chicken Casserole approx.30 minutes prior to calling 911 <br /> E: The patient had just finished eating a 3-day old chicken casserole when he began to feel "slightly nauseated". Patient <br /> wentto the bathroom and vomited once.The patient apologizes to EMS for being called to his house and states that he <br /> "feels fine".The wife of the patient looks at you and states, "I know my husband and something is just not right.He never <br /> gets sick.Maybe it was the casserole.Please help him." <br /> 1 OPQRST History Obtained <br /> O: 19:30—sudden onset—no signs or symptoms prior to acute onset <br /> P: Patient was sitting and resting at onset of nausea.
