Yes 1-5) The residents were very clean and <br />appeared well cared for. We observed <br />them interacting with staff on several <br />levels—personal care, opinions, <br />friendly discussion. They interacted as <br />possible with our committee. The <br />staff interactions we observed were <br />appropriate and high quality with staff <br />meeting the residents at their function <br />level. <br />2. Did residents say they receive assistance with personal care <br />activities? (i.e. brushing their teeth, combing their hair, <br />inserting dentures or cleaning their eyeglasses) <br /> Yes <br />3. Did you see or hear residents being encouraged to participate in <br />their care by staff members? Yes <br />4. Were residents interacting with staff, other residents & visitors? Yes <br />5. The residents on site at our visit are <br />living with dementia and describing <br />their environment was not likely, <br />however, they were smiling, friendly <br />and appeared comfortable, secure and <br />relaxed. <br />11. Yorktown is very quiet. <br />13. Buttons in the bathrooms and <br />bells available for those that want/need <br />them. Unfortunately, most residents <br />are no longer capable of using the <br />bells to get help. Yorktown has bed <br />and chair alarms for those that need <br />them. <br /> 9.
