<br /> <br />Facility / date: The Stratford 2/23/17 <br /> <br />Areas of Concern Exit Summary <br />Are there resident issues or topics that need follow-up or <br />review at a later time or during the next visit? <br /> Limited use of staff name tags. <br /> <br /> <br />Discuss items from “Areas of Concern” Section as well as <br />any changes observed during the visit. Give summary of <br />visit with Administrator or SIC. Does the facility have <br />needs that the committee or community could help <br />address? <br /> Two committee members met with residents during this <br />visit, while another met with the Resident Care <br />Coordinator and two representatives from the Orange <br />Co. Department on Aging. The purpose of this meeting <br />was to gauge the interest of the facility in participating <br />in DoA offered educational programming, namely the <br />Falls Prevention and/or Brain Health Academy. The <br />facility staff expressed considerable interested and <br />indicated that they would expect a good turnout of <br />residents for these six-week programs. DoA reps have <br />followed up with the administrator and are aiming for <br />spring to initiate one of the Academies. <br /> <br />Pertinent to the visit itself, the committee observed <br />continued progress observed in Memory Care re: staff <br />interactions with residents. Overall, staff were cordial <br />and visible. The facility was clean and pleasantly <br />decorated for Valentine’s Day. Residents engaging <br />with each other and with CAC visitors about the <br />birthday party that just concluded, including a <br />celebration in Memory Care. Residents waiting in <br />foyer were anticipating the evening meal. <br /> <br />
