More residents were visible and <br />actively walking down the halls than <br />previous visits. <br />2. The resident (with restricted <br />movement) who shared at the last visit <br />that he is unable to obtain assistance <br />during the night to empty his urinal <br />indicates that there has been no <br />improvement <br /> 4. More staff members were seen at <br />this visit and some were assisting <br />residents with walkers. <br />5a. No staff members had name tags. <br />“There aren’t any. It’s just not done <br />here” per the new administrator. <br />2. Did residents say they receive assistance with personal care <br />activities? Did residents describe their living environment as homelike? No 8. A bedroom on the men’s hall was <br />being evacuated due to bedbugs. The <br />room was in “isolation”. The State <br />guidelines were being followed and <br />many room fixtures were already in the <br />‘hot box’ being treated. The two <br />affected men were to be showered and <br />moved to a different room until the <br />issue was resolved. <br />CAC members noted that the <br />temperature and humidity in this wing <br />was uncomfortably high, though no <br /> 9.
