Most residents were cleanly dressed <br />with appropriate attire for the weather. <br />Two women walking down a hall had <br />a strong urine odor. <br />2. One resident (with limited <br />movement) who wished to remain <br />anonymous indicated that he is unable <br />to obtain assistance during the night <br />to empty his urinal. He then has no <br />choice but to wet the bed and remains <br />so until the morning. He does not have <br />a ‘call bell’ and if he calls out, no one <br />comes. He also reports that his <br />wheelchair tipped over last week when <br />he was outside. No one at the facility <br />noted this but he was assisted by <br />passing motorists who stopped to help <br />him. He is aware of his resident’s <br />rights and has the ombudsman phone <br />number memorized. He indicates that <br />when he calls, the phone just rings and <br />no one ever answers. <br />3. The same resident as above <br />indicates that staff, rather than <br />‘encouraging’ him to participate in his <br />own care, demands that he does so, <br />though he is physically incapable of <br />completing the task without assistance. <br />4. Very few staff members were seen, <br />except when notifying residents to <br />collect for dinner. One staff observed <br />passing meds in the dining room; <br />another doing laundry. <br />2.
