Activities were posted for <br />three days handwritten, in large <br />font on the bulletin board outside <br />the dining room. <br />17. Meals are planned, and portions <br />controlled according to regulations. <br />Meal choice is limited, however the <br />people visited today did not have <br />complaints. We observed the <br />afternoon snack cart sharing snacks. <br />NOTE: Crescent Green is an <br />“open facility” and many residents <br />go offsite where they may purchase <br />food and beverage choices. <br />17c. It was resolved quickly but raises a <br />possible facility security issue. <br /> <br />Although the Director states Crescent Green is not a locked <br />Memory Facility, when talking with residents, various <br />cognitive challenges are evident. She said residents “sign in <br />and out.” They watch for wandering behaviors and if it <br />becomes a safety issue, residents are moved to an appropriate <br />facility. <br /> <br /> <br />Discuss items from “Areas of Concern” Section as well as <br />any changes observed during the visit. Give summary of <br />visit with Administrator or SIC. Does the facility have <br />needs that the committee or community could help <br />address? <br /> Doris Coleman was not on site, but on vacation. Yolanda <br />was open about Crescent Green and shared information <br />freely. <br /> <br />Due to shift change and a brief staff meeting, fewer staff <br />were observed in the public areas at today’s site visit.. <br /> <br />Yolanda said that limited funds are available for facility <br />improvements (e.g. painting and other improvements), but <br />they are working on improvements as available. We <br />noticed residents having difficulty entering and exiting <br />the building due to the heavy doors, mobility issues, <br />wheelchairs, and no automatic door openers. Yolanda <br />was interested in the possibility of an automatic <br />“handicap” door opener on one of the doorways. <br /> <br /> <br />
