15. Were residents asked their preferences or opinions about the activities
planned for them at the facility?
Yes 15. Comments from both staff and
residents indicate Carol Woods’ staff and
residents treat one another with respect
and courtesy and friendship. An activity
was finishing when we arrived, and
residents and staff were on the front
porch and lawn area at Bg 5, all engaged
and sharing observations and information
about the blooming magnolia.

Communication on matters of activities,
preferences, and food are shared.

Activity calendars and other
announcements posted for the benefit of
residents are posted in large font at a
comfortable height for reading and in a
cheerful manner.

Photos of staff on duty are displayed for
residents, guests, visitors with names
clearly printed.

17a. Two residents mentioned the
challenge of finding seats in the dining
hall for single people. The residents
wanted us to pass on the idea of round
Friendship Tables, which would
accompany 5-6 people and make it easier
for single residents to socialize with
groups of 2-4.

15a. Was a current activity calendar posted in the facility? Yes
15b. Were activities scheduled to occur at the time of your visit actually
occurring?
Yes
See
note
16. Do residents have the opportunity to purchase personal items of their
choice using their monthly needs funds?
N/A
16a. Can residents access their monthly needs funds at their convenience?
(#16 and 16a pertain only to residents on Medicaid/Special
Assistance. NHs $30 per month. ACHs $66 minus medication
co-pay and full cost OTC drugs)
N/A
17. Are residents asked their preferences about meal & snack choices?
(Adult Care Home residents should receive snacks 3X per day.
Nursing Home residents should be offered snacks at bedtime.)
Yes
17a. Are they given a choice about where they prefer to dine? Yes*
17b. Did residents express positive opinions regarding their dining
experience (the food provided)?
Yes
17c. Is fresh ice water available and provided to residents? Yes
18. Do residents have privacy in making and receiving phone calls? Yes
19. Is there evidence of community involvement from other civic,
volunteer or religious groups?
Yes
20. Does the facility have a functioning: Resident’s Council?
Family Council?
Yes
Yes

Areas of Concern Exit Summary
Are there resident issues or topics that need follow-up or
review at a later time or during the next visit?

Simple questions were posed by residents, no serious concerns
or difficulties. Questions were shared with staff at the exit
interview and they took notes and assured us they would
follow up on each point..

It may be worth follow up on the “single diner” question for
seating to learn if Carol Woods has developed a positive
solution. At the Exit Interview, staff made note of the concern
and said they would keep working on possible solutions.
Discuss items from “Areas of Concern” Section as well as any
changes observed during the visit. Give summary of visit with
Administrator or SIC. Does the facility have needs that the
committee or community could help address?
______________________________________________

As at previous visits, Carol Wood presents a positive,
enriching, and caring home for its resident family. At the exit
interview, resident questions and comments were well received
and notes were taken to be sure they addressed any questions
passed along with individual residents. Staff knows their
resident family well.

Family involvement is very strong at Carol Woods, several
resident comments exemplified frequent visits, interactions
among family and staff, outings with family and calls

We were warmly greeted, welcomed and thanked for the
volunteer service to represent residents’ interests and needs.
