A proposed field services schedule is presented below. <br />Results of the 483DP -2A delineation phase activities will be incorporated in the Task Order 483DP -3 report <br />(previously authorized). S &ME anticipates completing the corresponding delineation phase report two weeks <br />after the field work has concluded. <br />Project Budget <br />5 &ME proposes to provide the Scope of Services presented above on a time and materials basis for an estimated <br />fee of $17,525.70. See the attached spreadsheet for details of the proposed budget. <br />,;,> Authorization <br />Our proposed services will be performed according to the terms of the Consulting Services Agreement, dated <br />August 1, 2017, between the Orange County and S&ME. For Orange County to be eligible for reimbursement for <br />the proposed services, we understand Orange County will submit our proposed work plan and cost estimate to <br />NCDEQ for preapproval. After receiving approval from NCDEQ we understand Orange County will issue a <br />Purchase Order as acceptance of this proposed work plan and cost estimate. Please reference this change order <br />proposal number and date (Change Order CO -2, July 25, 2018) on the Purchase Order. Upon receipt of Orange <br />County's Purchase Order, we will then proceed with the performance of the services. Your purchase order will be <br />an acceptance of the Consulting Services Agreement and an authorization to proceed with the performance of our <br />services. The terms and conditions included in any purchase order shall not apply, as our agreement is for services <br />that are not compatible with purchase order agreements. <br />July 25, 2018 <br />
