DocuSign Envelope ID: 717C8AE2- 4D6E- 4E46- A9A7- F4213973D039 <br />EXHIBIT A: PROVIDER'S OUTSIDE AGENCY APPLICATION <br />Program Goal <br />Club Nova will address food insecurity and diet related disease by <br />providing at least 12,000 healthy, affordable meals, engaging at least <br />80`Y of membership in Clubhouse meals, and providing education and <br />training related to cooking, nutrition, obtaining and growing food. <br />Performance Measures <br />Club Nova will: <br />• Maintain records of the number of meals served daily. <br />• Maintain daily records of individuals eating at the Clubhouse. <br />• Survey its membership yearly to evaluate the achieved health and food <br />security benefits of our healthy, affordable meal program and nutrition <br />education. <br />• Staff will closely monitor specific individuals' wellness goals through <br />progress notes. <br />Previous Year Program Results <br />100% of Club Nova members had access to an average of 13 healthy, <br />well- balanced meals each week (5 breakfasts, 6 lunches and 2 dinners), <br />with 12,015 meals served for the year. <br />85% of Club Nova members participated in meals. <br />This service led to: <br />• Decreased food insecurity for members <br />• increased knowledge of culinary skills, nutrition, and gardening <br />• Increased knowledge in the health benefits related to a healthy diet <br />• Decreased incidence of nutrition - related diseases <br />Current Year Estimated Results <br />100% of Club Nova members will have access to an average of 13 <br />healthy, well- balanced meals each week (5 breakfasts, 6 lunches and 2 <br />dinners), with an estimated 12,100 meals served for the year. <br />An estimated 85% of Club Nova members will participate in meals. <br />This service will lead to: <br />• Decrease in food insecurity for members <br />• Increase in knowledge of culinary skills, nutrition, and gardening <br />• Increase in knowledge in the health benefits related to a healthy diet <br />• Decrease in incidence of nutrition- related diseases <br />Next Year Projected Results <br />100% of Club Nova members will have access to an average of 13 <br />healthy, well- balanced meals each week (5 breakfasts, 6 lunches and 2 <br />dinners), with 12,200 meals served for the year. <br />At least 85% of Club Nova members will participate in meals. <br />This service will lead to: <br />• Decrease in food insecurity for members <br />• Increase in knowledge of culinary skills, nutrition, and gardening <br />• Increase in knowledge in the health benefits related to a healthy diet <br />• Decrease in incidence of nutrition- related diseases <br />PROGRAM INFORMATION 1/23/2018 1:58:29 PM P a g e 2 7 of 3 1 <br />
