AGENCY INFORMATION (Be Very Brief and Concise) <br />Please provide the following information about your agency (2 pages OR LESS): <br />a) Years in Operation, Date of Incorporation (Month/Year): 11 years in operation; 0212007 <br />b) Agency's Purpose /Mission (no more than a few sentences): Bridge Il Sports (BIIS), <br />mission is to educate, develop, and implement opportunities for youth and adults with <br />physical disabilities to play individual, team and recreational sports developing <br />independence and inclusion. <br />c) Types of Services the Agency Provides (bullet format): <br />• Adapted sport programming to children and adults with physical disabilities <br />• Trainings, clinics, competitions to engage athletes and coaches in higher levels <br />of competitive play <br />• Statewide school initiative educating students about acceptance, compassion, <br />and inclusive play <br />• Disability etiquette /diversity in the workplace trainings for businesses and <br />corporations <br />• Municipal and university partnerships to empower communities on the <br />importance and need of accessibility <br />d) Agency's History with Providing These Services: 11 years of a variety of adapted <br />sports Programming provides seasonal recreation opportunities for individuals with <br />disabilities in Orange County. Athletes and community members experience adapted <br />practices, clinics, trainings and competitions. Our partnerships with municipality <br />groups and UNC- Chapel Hill grows our programs within the County, simultaneously <br />maintaining sustainability. <br />e) Other Pertinent Agency Information (Ex. Has the agency experienced any major <br />changes in the past year? is there a new Executive Director? Are there new <br />initiatives ?) BIIS implemented the #EverybodyPlaysNC educational initiative in June <br />2016, helping to serve over 5,000 students across NC. Surveys are distributed to gain <br />insight into students' views, illustrating the learned concepts towards acceptance of <br />disability. <br />f) Schedule of Positions (For Entire Agency) <br />• Full Time Equivalent (FTE) staff will be noted as 1.00; half time as .50; quarter time as .25, etc. <br />• Calculate a Full Time Equivalent for all recorded volunteer hours using the following: <br />Total Volunteer Hours = Volunteer FTE <br />2,080 <br /># of FTE - Full -Time Paid Positions: 8 <br /># of FTE - Paid Part -Time Positions: 2 <br /># of Volunteers: 1,250 <br />g) Living Wage <br /># of FTE - Volunteers: 125 <br />Does this agency pay permanent employees a minimum living wage?
