If you use percentages, you must also 2rovide the total number of particiVants <br />within that measure's description or for an earlier performance measure. <br />Performance Measures <br />Anticipated <br />Results <br />PARTNER ABUSE INTERVENTION: Track referrals to Pathways to Change vis -A -vis online <br />65 referrals <br />referral system, new volunteer court liaison for abuser accountability program (in court <br />for PAI <br />appearances), presentations to various law enforcement agencies; and telephone referrals, etc. All <br />referrals will be maintained in Pathways database which contains referrer name, contact, and <br />demographic information about person being referred. <br />PARTNER ABUSE INTERVENTION: Pathways will conduct 52 assessments on those <br />52 <br />referred to the PAI program. These are in person interviews where the potential participant is <br />assessments <br />engaged in an intensive one -hour interview, including lethality assessment, etc. This will be <br />conducted <br />tracked via intake forms that are uploaded to Pathways Team Drive which is a Google -based <br />platform. <br />PARTNER ABUSE INTERVENTION: Seventeen (17) participants will successfully complete <br />17 <br />Pathways 26 -week Partner Abuse Intervention Program. Attendance is tracked by facilitators on <br />successful <br />a Team Drive that is a Google -based platform. Cases are normally dismissed after completion of <br />PAI <br />the program, Because the course runs 26 -weeks it is unlikely that all of those referred, assessed <br />completions. <br />and enrolled will have completed the course at the conclusion of the 2018 -19 fiscal year. <br />ANGER MANAGEMENT: Track referrals to Pathways to Change vis -a -vis online referral <br />57 referrals <br />system, new volunteer court liaison for abuser accountability program (in court appearances), <br />to Anger <br />presentations to various law enforcement agencies; marketing via MailChimp automated software <br />Management <br />marketing software platform, and telephone referrals, etc. All referrals will be maintained in <br />Pathways database which contains referrer name, contact, and demographic information about <br />person being referred. <br />ANGER MANAGEMENT: Pathways will conduct forty -six (46) assessments on those referred <br />46 <br />to the Anger Management program.
