DocuSign Envelope ID: 74839431 - 9216- 429F- BFA4- 2AC6473BD9D3 <br />EXHIBIT 1111311 <br />Scope of Services — FY 2018 -19 <br />Outside Agency Performance Agreement <br />Agency Name: Pathways to Change <br />Program Name: Partner Abuse Intervention and Anger Management <br />Funding Award: $7,000.00 <br />Program Services <br />Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by June 30, 2019. <br />Pathways to Change will enroll participants in the following programs during fiscal year 20118 -19. <br />- 12 -Week ANGER MANAGEMENT PROGRAM: Our Emotional Intelligence & Anger Management <br />program offers participants an opportunity to explore triggers for their anger, discover the dynamics of <br />social and emotional competence, and replace unhealthy behaviors and communication styles with <br />respectful, controlled and assertive communication. <br />- 26 -Week PARTNER ABUSE INTERVENTION PROGRAM: Our Partner Abuse Intervention program <br />meets North Carolina requirements for Batterer Intervention or Abuser Treatment. Participation in a <br />program like this may be court- ordered after an individual is convicted of a domestic violence crime. <br />Others may be referred by social services, their church, or themselves.
