AGENCY INFORMATION (Be Very Brief and Concise) <br />Please provide the following information about your agency (2 pages OR LESS): <br />a) Years in Operation, Date of incorporation (Month/Year): 1 yeart 1 month: Incorporated in <br />November of 2016 <br />bj Agency's Purpose /Mission (no more than a few sentences): <br />In alliance with community partners listed below, QCRA builds awareness of the needs of <br />rural seniors and supports them with what they need to live independent and dignified lives <br />access to county services, safe homes, nutritious food, and friendly human contact. <br />OCRNs main mission towards this goal is to provide weekly hot, nutritious meals to rural <br />seniors in underserved areas of rural Grange County, <br />c) Types of Services the Agency Provides (bullet format): <br />• Nutritious meals <br />• Social contact <br />• Liaison with county services <br />d) Agency's History with Providing These Services: <br />QCRA evolved from the SALT /Project EngAGE initiative who discovered that many seniors <br />in rural Orange County were in need of better nutrition and having regular personal contact <br />with someone. Through regular visits over the last 30 months, volunteers have created <br />personal relationships and trust with their "clients ". Through that relationship it was clear <br />that some clients would benefit from nutritious, hot meals on a regular basis. <br />In their beginning months, fOCRA provided meals to 22 of those seniors, QCRA started a <br />formal meal delivery program in November of 2016 with volunteers /caterers donating meals <br />for 40 rural seniors one time per week. As of December 20117 we are serving 82 seniors <br />one hot meal a week, and a pilot second meal is being established. To date, QCRA has <br />served nearly 4,000 meals, including a bonus Thanksgiving meal. Emergency shelf stable <br />meals are now available and have been delivered to 82 people. <br />We are in the process of scaling up, with a long term goal of delivering 5 hot meals a week <br />to seniors in the rural, unincorporated parts of Orange County who are outside the reach of <br />other Meals on Wheels programs and are in need of our help. As QCRA is evolving, our <br />short term goal is to deliver 2 hot meals a week by the end of 2018, <br />e) Other Pertinent Agency information (Ex. Has the agency experienced any major changes <br />in the past year2 is there a new Executive Director? Are there new initiatives ?) <br />This year has seen an increase in partner alliances and collaborations, the start -up of an <br />emergency food pantry, and the distribution of emergency shelf stable meals. CCRA also is <br />participating in the Commodity Surplus Food Program (CSFP) in alliance with the Dept of <br />Aging.
