AGENCY INFORMATION (Be Very Brief and Concise) <br />Please provide the following information about your agency (2 pages OR LESS): <br />a) Years in Operation, Date of Incorporation (Month/Year): <br />41 years in operation, October 1976 <br />b) Agency's Purpose /Mission (no more than a few sentences): <br />We nourish the bodies and spirits of the homebound with a balanced meal and the <br />human connection they need to help them live independently. We provide a hot meal <br />delivered to the home every weekday by a corps of volunteers. Resource and <br />referral services as needed. <br />c) Types of Services the Agency Provides (bullet format): <br />• M -F lunch delivery, <br />• social visit, <br />• wellness check -in, <br />• monthly porch bag delivery (shelf stable food), <br />• deliver birthday cards, pet food, holiday gifts, birthday flowers, and letters, art <br />projects and gifts from local children & community members. <br />d) Agency's History with Providing These Services: <br />For 41 years, Chapel Hill Carrboro Meals on Wheels has provided daily weekday <br />meals and a social visit to the homebound, elderly, those convalescing and to the <br />physically or mentally handicapped in need of our services. Service is provided out <br />of Binkley Baptist Church which has generously donated its space to ensure our <br />funds can serve those in need. Over the past ten years we have steadily increased <br />the numbers of clients served to approximately 280, and have expanded our delivery <br />area to meet the growing need of the underserved populations in the county. We are <br />committed to serve our clients in the most meaningful and efficient way possible and <br />thus continually strive for improvement through feedback from our clients and <br />stakeholders. We distribute an annual client satisfaction survey which consistently <br />receives overwhelmingly positive results and this past year we conducted a <br />stakeholder planning survey to gather input which could help us improve. We are <br />members of both the statewide and national Meals on Wheels associations and <br />consistently partner with our peers in neighboring counties to better serve our <br />clientele. We are currently working together to find alternate sources of funding to <br />help with the cost of providing services to increasing numbers of clients requesting <br />services. <br />e) Other Pertinent Agency Information (Ex. Has the agency experienced any major changes <br />in the past year? Is there a new Executive Director? Are there new initiatives ?) <br />CHCMOW directly impacts the lives of those who are frail, disabled, and or <br />homebound by providing food assistance, social and emotional support and <br />information about other area services to those who are often also in economic <br />distress. In 2017, thanks to the hard work of our more than 250 dedicated volunteer <br />drivers and bakers, CHCMOW provided over 46,000 meals. We serve a diverse <br />client base that is 43% minority, 66% female, and increasingly more elderly. 29% of <br />our clients last year were 85 years of age or older. Our dedicated volunteers now <br />deliver on 14 routes Monday through Friday all the way west to the Alamance <br />border, east to Durham, south to Chatham and north to Mount Sinai Road with a <br />"buffer area" where we call OCIM to determine which program has a route that is <br />Agency Information 1/22/2018 9:22:00 AM Page 10 of 26 <br />
