DocuSign Envelope ID: 1640431D- 872F- 4BC9- ACA1- 2226D9788920 <br />EXHIBIT A: PROVIDER'S OUTSIDE AGENCY APPLICATION <br />1) What efforts do you make to seek feedback about your program from your target population <br />(e.g. survey, evaluations, etc. ?) <br />Data is collected through intake interviews and end of year interpreted interviews with each <br />family. <br />m) Include any other pertinent information. <br />For the first half of FY18, highlights include: <br />• TTCF donated 1,498 pounds of food grown by TTCF farmers to PORCH for an <br />average of 124 refugee families during first and second quarter. <br />6 5 farmers grew for 150 CSA members for the first and second quarter <br />100% of farmers report decrease in food insecurity based on information gathered <br />during 2017 Year End interviews conducted in December of 2017. <br />• Teens involved in TTCF's youth program provided community cooking <br />demonstrations, tours of the farm, planned initiated and implemented farm /culture <br />focused events, continued documentary work focused on cultural traditions on <br />farmers stories and traditions from Burma, collaborated with other youth working in <br />food justice in NC, attended conferences and workshops throughout the state and <br />served in advisory capacity for TTCF project. Eleven teens regularly engaged with <br />130 event and workshop hours during first half of FY18. Additionally, teens <br />regularly engage in bigger picture work that relates to food and social justice. Most <br />recent accomplishments include the youth led national Rooted in Community <br />conference co- hosted by Transplanting Traditions. Additionally, with the support of <br />Chapel Hill Carrboro Youth Forward- Bridging the Gap grant, Rea Rea Wei, a past <br />youth participant was hired in 2017 to work as the Refugee Youth Program <br />Coordinator. Rea Rea began attending Guilford college on as a Bonner Scholar <br />this Fall and her work with TTCF was central to her Bonner application. <br />From July to December, TTCF staff and TTCF youth program participants provided <br />18 tours to 313 individuals from local and national entities including; universities, <br />public school classes, community organizations, funders and businesses. <br />TTCF staff provided two on -farm workshops per week from July to October. Each <br />workshop lasted approximately one hour. Workshops included weekly CSA farmer <br />meetings and weekly agricultural focused workshops open to all levels of farmers. <br />Additionally, farmers participated in three, half -day field trips to established area <br />farms to learn specific agricultural techniques. <br />• In November 2017, OCPYC received a GSK Impact Award for our work with <br />Transplanting Traditions Community Farm and Growing Up Healthy, including our <br />farm to child care activities. <br />PROGRAM INFORMATION 1/22/2018 6:18:22 PM age 15 of 22 <br />
