DocuSign Envelope ID: FOFFD659- 4159- 4D4C- 82FD- FF8E51A5A7E4 <br />EXHIBIT A: PROVIDER'S OUTSIDE AGENCY APPLICATION <br />• Orange County library, Chapel Hill Public Library, school libraries and book sellers: <br />collaborate on Women's History Month and Domestic Violence Awareness Month <br />events. <br />• North Carolina Coalition Against Domestic Violence: Collaborate to offer <br />professional trainings on interpersonal violence and provide staff training on <br />evidence -based practices. <br />• NC Diaper Bank: through this partnership we provided over 42,000 diapers in 2017. <br />Compass Center is the only Diaper Bank community partner in Orange County that <br />distributes diapers to any county resident in need. Safety net programs such as WIC <br />and food stamps do not cover diapers, often leaving families with a difficult decision <br />between buying diapers and other necessities with limited funds. <br />• Numerous non- profit and government agencies and community groups request our <br />professional and community education programming for their constituents to be able <br />to better respond to their own clients or situations. <br />Program Description (3 pages OR LESS) <br />Phase provide the following information about the proposed program: <br />d) Summarize the program services proposed and how the program will address a <br />Town /County priority /goal? <br />Our Information and Referral Services program works to assess the needs of clients in- <br />person or over the phone and connect them with a host of referrals to service providers to <br />meet their expressed needs (most common issues: housing, low -cost mental <br />health /medical services, legal problems, child care, financial insecurity, etc.) in addition to <br />Compass Center services. Being a leader in referral best practices and quality control has <br />been of particular focus to us in the last three years, and after an assessment and <br />restructuring of these services we have created greater staff support and service <br />consistency. Last year we served 922 individuals in 2,183 contacts in this program. In order <br />to keep up with demand and maintain quality services we partnered with Johnson Service <br />Corps to have an intern who oversees the program as an Information and Referral <br />Specialist and also assists other programs to meet client needs. <br />The Community Education component of our program addresses the need for youth and <br />adult training and community education around issues of interpersonal violence prevention <br />and response by delivering trainings and workshops to build skills and knowledge, <br />disseminating information through various channels. Start Strong, our teen dating violence <br />prevention program, is a two- session program delivered to 6th and 8th grade students during <br />health class in Chapel Hill - Carrboro City Schools. We also offer professional domestic <br />violence education events to a wide array of community members including law <br />enforcement, attorneys, medical providers, other non - profit service providers, faith leaders, <br />and civic organizations. Our staff and volunteers provide information at many community <br />events and perform other outreach activities to educate our community about issues related <br />to domestic violence, adolescent health, and agency and community services. <br />The Civic Engagement component of our program includes coordinating community <br />education events and activity calendars for Domestic Violence Awareness Month and <br />Women's History Month. <br />PROGRAM INFORMATION 1/22/2018 5:50 :18 PM Page 3 5 of 47 <br />
