DocuSign Envelope ID: FOFFD659- 4159- 4D4C- 82FD- FF8E51A5A7E4 <br />EXHIBIT A: PROVIDER'S OUTSIDE AGENCY APPLICATION <br />Center also offers Domestic Violence Crisis Services and we are re- launching our <br />Career and Financial services to make them more trauma - informed, supporting <br />financial, career and legal resources has the potential to save lives. <br />j) Describe what would happen if requested funding is not awarded at all or if a reduced <br />allocation is recommended. <br />We have a very long history of providing these services to the community. They were <br />partially funded for many years by the NC Council for Women through Displaced <br />Homemaker funds (at one point over $60,000 annually), but as of July 1, 2014 Displaced <br />Homemaker funds no longer exist. We were forced to reduce our Self - Sufficiency staff <br />three years ago from two to one. After setting the goals of increasing the accessibility of our <br />Career and Financial Services overall, particularly for lower- income clients and/or victims of <br />domestic violence, and growing both the number of participants and the number of <br />returning clients, and after conducting work with consultants to re- evaluate our Career and <br />Financial Services, this work has confirmed that to take the next step and fully launch our <br />program improvement and expansion plan successfully would require a second full -time <br />staff member. Expanding Career and Financial Services will enable us to provide more <br />career and financial workshops and individual counselling sessions, increase the variety <br />and frequency of topics covered, offer more evening and weekend fours for sessions, <br />expand our volunteer training to significantly increase the number of Career and Financial <br />volunteers, forge new partnerships with other organizations with self - sufficiency expertise, <br />offer incentives to participating clients including free childcare and transportation support, <br />and would mean we could serve more Career and Financial clients overall. Additional staff <br />capacity would also support us as we work to conduct long -term evaluation of the Career <br />and Financial Services redesign, to track the growth of services and measure the impact on <br />clients' long -term financial self- sufficiency. <br />We will continue to seek alternative funding and expanded collaborations to help support <br />these services, but we need support from Orange County while we launch the expansion of <br />these services, since we have found that many funders prefer to support existing rather <br />than new initiatives. A loss of funding from Orange County governments would make it <br />difficult for us to sustain funding the current primary staff member for these programs, even <br />though they are an essential component to the continuum of services that we provide to our <br />clients, and would force us to not fully implement the program expansion plan. <br />k) What percentage of your target population is low - moderate income? <br />We estimate that over 80% of the target population is low- moderate income. <br />1) What efforts do you make to seek feedback about your program from your target population <br />(e.g. survey, evaluations, etc. ?) <br />Clients are regularly asked to complete anonymous surveys that are program - specific to <br />track outcome measures, assess satisfaction with services, and gain client feedback. <br />These survey results are reviewed regularly by program directors and at least quarterly by <br />the Associate Director. <br />m) Include any other pertinent information. <br />PROGRAM INFORMATION 1/23/2018'10:28:52 AM Page 29 of 47 <br />
