PUBLIC HEALTH TASK FORCE 2004: PRELIMINARY RECOMMENDATIONS <br />PUBLIC HEALTH IMPROVEMENT PLAN <br />Planning & Outcomes Committee <br />13. Fund local health departments to assess and document community health needs through <br />community partnerships. Page 28 <br />Establish a uniform statewide process for community health assessment to be conducted on a <br />four -year cycle (Comprehensive Community Health Assessment) and updated annually <br />(State of the County Report). $ 1,623,000 <br />=> Establish a core set of questions to be used for primary data collection statewide. Local <br />partnerships (Healthy Carolinians, Community -based Organizations, and other health <br />agencies) may develop additional questions according to their needs. <br />=> Build the capacity of the state Office of Healthy Carolinians to support local <br />community assessment through local training, technical assistance, and report <br />generation. $ 441,000 <br />14. Establish a process for comprehensive collaborative planning that integrates state and local <br />needs assessment, priorities, and strategic program objectives. Page 29 <br />=> Develop and implement a collaborative State Public Health Plan to cover four years and <br />updated annually. <br />State Center for Health Statistics will provide county specific health data to local health <br />agencies for the purpose of local planning and priority setting. <br />The Office of Healthy Carolinians will compile and report information on local needs, <br />community priorities, and action plans to state level programs. <br />Establish an annual integrated planning cycle to inform state and local decision makers <br />regarding program priorities and funding allocations. <br />15. Fund increased information technology capacity at the local level to collect, compile, <br />analyze, and report essential public health data. Page 30 <br />=> Build local capacity to collect, analyze, and report critical public health information <br />electronically. $5,160,000 <br />=> Assure compliance with HIPAA guidelines. <br />Build the local interface with the Public Health Information Network to enhance the <br />ability of local health departments, hospitals, healthcare providers, and community <br />partners to communicate electronically in a secure environment. $ 860,000 (one time <br />equipment) <br />7 <br />
