DocuSign Envelope ID: 97E04238- A5D4- 41EE- ACB6- DDF028BB5823 <br />The combination of professional, accurate document imaging and 29 years of experience in file <br />management is a distinct advantage that Starpoint can offer the Orange County Health <br />Department. <br />Scope of Work <br />All- Inclusive Intake and Inventory <br />• Starpoint personnel will pack and relocate containers of hardcopy records from the locked <br />cabinets at the Hillsborough clinic (a breakdown by record type is included in the cost <br />section). There is no additional cost for labor, intake or transport of these records. For <br />Orange County, the cost of empty containers needed for transport will also be waived, <br />representing hundreds of dollars in savings for the County. <br />• Already boxed containers from the storage cages will be reviewed one by one with clinic <br />staff to determine which boxes will be removed for scanning. Starpoint will barcode the <br />targeted containers and will define contents lists prior to relocation to our facility. We will <br />leave behind all boxes that are already eligible for destruction or which will be eligible <br />within the next few years. <br />• Starpoint's barcode and transmittal process will ensure that all containers are tracked both <br />physically and by reported activity from the moment they are picked up, throughout their <br />lifecycle. <br />• Starpoint will physically or digitally deliver any chart needed by the Health Department <br />during normal business hours for the duration of the scanning process. <br />• Starpoint will establish authorized access by Health Department users. <br />Digital Conversion <br />• Starpoint will purge and digitally convert any records that Orange County Health <br />Department wishes to digitize at the all- inclusive rate of 4.2 cents per image. <br />• Charts will be prepped for feeding into the scanners by removing any paper clips, staples <br />and post -it notes, and copying and repairing damaged pages whenever required. <br />• Starpoint will image all pages at full duplex mode with our Ultrasonic Multi -Feed Detection <br />equipped Bowe Bell + Howell scanners to ensure a 100% capture rate. <br />• Images will be scanned at 200 dpi in TIFF Group N format files for interoperability. <br />Following scanning, a set of image and manipulation processes will be run to remove blank <br />pages, correct skewed images, and remove black borders. <br />• All images will be manually Quality Controlled to ensure total data capture and legibility. <br />Documents will be cross compared between physical and digital, with any inadequacies <br />immediately rectified through re- scanning. <br />• Starpoint will index all images using at a minimum patient names, dates of birth and static <br />fields to designate document types. <br />• Starpoint will process all files using Adobe OCR to enable full -text searchability at no <br />added cost. <br />
